What is embryo grading?

If your clinic just sent you a report with something like "4AA" or "3BC" next to your embryos, those letters and numbers are a description of how the embryo looks under a microscope on one specific day. They're a helpful clue, but please don’t take them as a final verdict because they’re not a prediction of whether you'll get pregnant.

What those letters and numbers mean

A lot of people see a "lower" grade and panic. But you have to think of it like a report card for how an embryo is developing, rather than a pass/fail grade.This scoring system, called the Gardner scale, is the one most fertility labs use.

For example, by day 5 or 6, embryos reach what's called the blastocyst stage, and this is when you'll usually see a code like 4AA. It's made up of three parts:

  • The number (1 to 6): how much the embryo has grown and expanded. Higher usually means it's developing on schedule.
  • The first letter (A, B, or C): the quality of the cells that will go on to become the baby.
  • The second letter (A, B, or C): the quality of the cells that will go on to become the placenta.

So a 4AA means: good growth, top-quality baby-forming cells, top-quality placenta-forming cells. A 3BC means: a bit less growth, mid-quality baby-forming cells, lower-quality placenta-forming.

Does a "better" grade mean a better chance of pregnancy?

Somewhat, but not as much as people assume. Here's what the research shows:

So grade is one useful clue, especially the placenta-cell score and timing, but don’t jump to conclusions based on just that. A lot of what determines success isn't visible in a grade at all.

Why a "lower grade" embryo can still absolutely work

This part often gets glossed over: embryos with lower grades are still fully capable of implanting and turning into a healthy pregnancy. Grading is also just one person's visual read on one day instead of being an exact science. Two different embryologists looking at the same embryo can sometimes give it different grades, simply because it comes down to trained judgment. 

So if your embryo isn't a straight-A report card, that is not the same thing as bad news.

Grading vs. genetic testing, what's the difference

Grading is about how an embryo looks. Genetic testing (called PGT-A) is a separate, more direct check of whether the embryo has the right number of chromosomes. They tell you different things, and neither one alone gives you the full picture.

Genetic testing isn't automatically the right call for everyone, either. A major clinical trial found that choosing embryos by appearance alone (no genetic testing) led to pregnancy outcomes that were just as good, overall, as using genetic testing to choose. That said, fertility specialists' own guidance points out that genetic testing does seem to meaningfully help specific groups, especially patients between 38 and 43.

So whether testing is worth it for you depends on your age and history, there's no one right answer for everyone.

What to do with your report

Your embryo grade is one piece of information your clinical team looks at, along with your age, history, and any testing you've had. It was never meant to be read on its own, and it's definitely not something to immediately panic over. 

If you're staring at a report full of letters and numbers you don't understand, the best approach is to just ask your clinical team to walk you through what it means for your specific situation.

What happens next: embryo transfers

Once your embryos are graded, the next step is deciding which one gets transferred first, and this is where it helps to understand what a "transfer" involves. A transfer is the procedure where an embryo is placed into the uterus, and it can happen one of two ways. A fresh transfer happens just three to five days after the egg retrieval, using the embryo right away. A frozen embryo transfer (FET) uses an embryo that's been frozen (sometimes for genetic testing, sometimes just to let the body recover from retrieval) and thawed later, on its own separate cycle. Frozen transfers have become the more common choice in the US, now making up over 75% of embryo transfers, up from about 30% a decade ago, largely because giving the body time to recover from retrieval before transferring tends to lead to a more receptive uterine lining. It’s common to need more than one transfer before a pregnancy takes place, which is why having multiple embryos frozen and graded in the first place matters: it means you can attempt additional transfers without repeating a full egg retrieval each time.

So the question is not just "what does my embryo's grade mean," but "how many transfers might it realistically take, and what happens if the first one doesn't work."
