Male Fertility 101: What a Sperm Analysis Can Tell You
If you're trying to conceive and things aren't moving as fast as you hoped, there's a good chance the answer lives on the male side of the equation. Male factors are involved in roughly half of all infertility cases worldwide, yet men are far less likely to get tested early. One study found that only 41% of Ob/Gyns even consider a urology referral for the male partner, and fewer than a quarter routinely order one before starting treatment.
Luckily though, a semen analysis is relatively inexpensive, non-invasive, and often the fastest way to find out what's going on. Here's what it measures, what the numbers mean, and what you can do if yours come back outside the normal range.
What is a male fertility test?
The standard test is a semen analysis: a lab exam of a semen sample that measures a handful of specific parameters, most commonly volume, sperm concentration, motility, morphology, and vitality. Together, these numbers describe how much semen you produce, how many sperm are in it, how well they move, and what shape they're in.
It’s important to note that a semen analysis is not a genetic test. It doesn't assess the genetic integrity inside the sperm, which is why some men with a "normal" result still face unexplained infertility, and why additional testing (like sperm DNA fragmentation) sometimes gets added later.
What is a “normal” result?
In 2021, the World Health Organization released its sixth-edition manual for semen analysis, updating the reference ranges labs use worldwide. The current lower reference limits are:
- Sperm concentration: 16 million per milliliter
- Total motility: 42%
- Progressive motility: 30%
- Normal morphology: 4% (using strict Kruger criteria)
A few important caveats. These numbers are the 5th percentile of a reference group of fertile men whose partners conceived naturally within a year, so they're a statistical benchmark, not a strict pass/fail line. A single test below the reference range isn't a diagnosis. Because sperm takes about 72 days to fully develop, a repeat test 4 to 8 weeks later is standard practice, and both the AUA and ASRM recommend at least two analyses before drawing conclusions, ideally about a month apart.
When should you get tested?
Current clinical guidelines from the AUA and ASRM recommend a semen analysis as part of the initial evaluation any time a couple is being worked up for infertility, not as an afterthought once the female partner's testing comes back clear. The guidelines are also explicit that couple infertility can stem from male factors, female factors, or both, and that evaluating one partner in isolation isn't appropriate. If you've had two or more failed IUI or IVF cycles, or recurrent pregnancy loss, that's also a clear signal to get the male partner evaluated if it hasn't happened already.
What affects sperm quality
Sperm production is more sensitive to daily habits than most people realize, and a lot of it is modifiable. The research consistently points to a handful of repeat offenders:
- Smoking: increases sperm DNA fragmentation by roughly 10% and is associated with reduced semen volume and sperm concentration in dose-dependent studies, with heavy smokers seeing the largest declines.
- Alcohol: chronic use raises DNA fragmentation by a similar margin, disrupts the hormone axis that drives sperm production, and can lead to testicular atrophy over time.
- Obesity: excess weight impairs sperm production through hormonal imbalance and inflammation, but even modest weight loss has been shown to improve semen parameters.
- Heat exposure: frequent sauna use, hot tubs, and prolonged laptop-on-lap time can raise testicular temperature enough to affect sperm production, and heat waves and other environmental exposures show up repeatedly in the research as a measurable factor.
- Sleep and stress: testosterone follows a daily rhythm, and disrupted sleep or chronic stress can throw that rhythm off, which in turn affects sperm production.
The encouraging part: because sperm regenerates roughly every 72 days, targeted changes (quitting smoking, cutting back on alcohol, addressing sleep, managing weight) can show up in a semen analysis within a few months, not years.
What happens if your numbers are outside the normal range
Depending on which parameters are affected, next steps might include a repeat test, bloodwork to check hormone levels, a physical exam for structural issues like varicocele, or a referral to a urologist or reproductive endocrinologist who specializes in male fertility. For couples moving into treatment, male factor findings also shape which path makes sense, whether that's IUI, IVF, or IVF with ICSI.
This is why testing both partners matters from the start. Waiting until female fertility workups are complete before considering the male side can add months to a process where time is often the thing people can least afford to lose.
